**Description**



**Required Qualifications: (As Evidenced by an Attached Resume)**



Associate's degree (equivalent) or higher. In lieu of degree, a combination of directly related education and/or experience totaling two (2) years may be considered. Experience in data collection, data entry and data tracking. Must be bi-lingual (English and Spanish) and computer literate. Must have reliable transportation. Candidate will be traveling to and from various sites several times a week.



**Preferred Qualifications:**



Bachelor's degree (equivalent) or higher in a health related field. Proficiency with MS Word and Excel. Familiarity with PowerChart (EMR). Previous experience working in a medical outpatient clinic.



**Brief Description of Duties:**



The Patient Navigator will serve as a point of contact and will be a resource for research subjects with mainly tick-borne diseases or other infectious diseases. Incumbent will be bi-lingual and possess excellent communication skills in both languages. Some travel to the eastern end of Long Island will be required to coordinate ongoing research studies (i.e., Riverhead, Greenport, East Hampton and Southampton).



+ Assist patients in understanding their diagnosis, treatment options, and the resources available.

+ Provide linkage to care with either their primary care provider, their primary specialist who treats tick-borne diseases and other infectious diseases or refer to providers at Stony Brook Medicine.

+ Follow patients through from diagnosis to first outpatient appointment.

+ Input information into database created by the data analyst and project manager.

+ Ensure timely acquisition of laboratory data and communication to all relevant parties.

+ Work with Data Analyst and Project Coordinator to collect data, track outcomes, and support strategic planning processes.

+ Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, diagnostic staff, nurses, laboratory staff and primary investigators.

+ Other duties as assigned.
